Preparedness. Surge Capacity. National Service.
U.S. Public Health Service Auxiliary
A proposed national volunteer auxiliary to strengthen America’s public health emergency response capability.
The United States needs trained PUBLIC HEALTH surge capacity that can be activated quickly during disasters, pandemics, and national emergencies.
Why America Needs This
Public health emergencies—including pandemics, hurricanes, and large-scale infrastructure failures—are increasing in frequency and complexity. These concurrent crises severely strain existing municipal and federal systems. A structured U.S. Public Health Service Auxiliary would provide the scalable manpower and professional expertise needed to support federal response operations, strengthening national resilience and surge capacity when it matters most.
What is the USPHS Auxiliary?
A Proposed Federally Aligned Volunteer Force for National Surge Capacity
The U.S. Public Health Service Auxiliary is a proposed federally aligned volunteer organization designed to augment the U.S. Public Health Service and HHS during major incidents and disasters. By organizing trained volunteers into a structured ready-force, the Auxiliary provides the scalable manpower needed to support federal response operations and strengthen national resilience.
Volunteers train locally and maintain readiness standards to ensure they are prepared for official activation when needed. Crucially, the USPHS Auxiliary does not replace USPHS commissioned officers; rather, it is designed to amplify their strategic impact through essential clinical, logistical, and administrative assistance during catastrophic emergencies.

Modeled After Proven Success
The U.S. Public Health Service Auxiliary proposal draws inspiration from established national volunteer corps that have strengthened American resilience for decades.
U.S. Coast Guard Auxiliary
Established in 1939, the Coast Guard Auxiliary provides a premier model for service integration. As a non-military volunteer force, it supports maritime safety and security missions, offering a blueprint for how an auxiliary can provide surge capacity to a uniformed service without replacing it.
Civil Air Patrol
As the official auxiliary of the U.S. Air Force, the Civil Air Patrol performs search and rescue missions, disaster relief support, and aerospace education. The CAP model proves that a volunteer organization, activated through government emergency mechanisms, provides a cost-effective and highly skilled surge capability for national response.
Modeled after successful national auxiliaries such as the U.S. Coast Guard Auxiliary and the U.S. Air Force Auxiliary (Civil Air Patrol).
